PD_PAL, Palliative Care in Parkinson's Disease - Evaluation of the combined and person-centered intervention of the Advance Care Planning (ACP) program for individuals with Parkinson's disease and their family caregivers

This multi-centre European project [1] is developing a new approach to care and support for people with Parkinson's disease by integrating a predictive care plan and new technologies into home care.
The aim is to validate a new model of palliative care that can be integrated into the traditional care system when mobility and independence become increasingly limited due to movement disorders. PD_Pal will provide an additional level of support to patients, their families and treating physicians, characterised by two main elements. Firstly, a specially trained nurse (PD_Pal Nurse) will empathically plan the desired end-of-life treatment together with the patient. On the other hand, there is the possibility of integrating new portable technology systems, the so-called PDMonitor® [2], which record the patient's movement parameters as part of the monitoring system. The PDMonitor® is designed to help clinicians record motor symptoms and manage patients with Parkinson's disease. Based on these two elements, the PD_Pal project contributes to the definition of new European standards for care concepts in the advanced phase of the disease.
